Community Health Programs

The Community Health Department provides a variety of services in the community.  The department’s focus is to provide outreach and education throughout the community on a wide range of health and wellness topics.  Some specific programs include:

  • Education and outreach: Providing education on a variety of health-related topics to community members of all ages, including:
  • Educational lunches and workshops with guest speakers
  • Nutrition education to Elders, Youth, Head Start and Make & Take program.
  • Physical activities such as community walks, Powwow Fun Run, snowshoe walks, Bingocize classes and Fit Family Fun Day.
  • Health Observances such as Great American Smoke Out and Breast Cancer awareness in October
  • Home Safety:  The focus is to promote safety for Elders in their homes.
  • Comprehensive home safety assessments can be completed by our trained staff members.
  • The Lifeline program (an in-home emergency alert program) is one of the services available to tribal members through Community Health.
  • Referrals for Lifeline only can be made to the Community Health Department also.  An application must be made and approved prior to installation of the Lifeline 
  • In-home Nursing Care:  The focus of this program is to help Elders safely remain in their homes as long as possible.  We offer the following services:
  • Medication Assistance
  • Our Registered Nurses can help set up your medications in a medication box- helping to explain your medications and make it easier to remember when and how to take your medications. 
  • The nurses can also assist with getting refills from the SMHWC Pharmacy and bring them to your home when they come to fill your medication box.
  • Post Hospital Care:
  • Let us know when you or a family member are admitted to the hospital.  We are not a Home Health Agency, but we can assist with hospital aftercare.  We can work with your Primary Care Provider and the hospital to make sure your needs are addressed.
  • With your permission, we will work in conjunction with the hospital discharge planner to make sure all necessary arrangements are made for when you come home.  
  • Care Coordination:
  • Assisting patient to stay on track with various appointments / specialist appointments and new medications.
  • Loan Closet: The SMHWC maintains a loan closet (for short term loan) of some Durable Medical Equipment (DME). 
  • Shower Chair, Toilet Seat Riser, Commodes, Walkers, Canes, Grab Bars for Showers, and Wheelchairs.  
  • Opioid Harm Reduction Programming:
  • Operate and maintain Public Health Vending Machine
  • Community education, awareness and outreach
  • Coalition building and community organizing
  • Drug Abatement activities
  • Food Stand Inspections:
  • To comply with Tribal ordinances, all food sales on the reservation need to be inspected, prior to food sales taking place and have a permit issued.  Please call to schedule that inspection.  One condition of the permit is to have food handlers’ certificate for anyone handling food.

  • Other services include:
  • Annual influenza clinics
  • Individual immunizations as necessary 
  • Car seat education, distribution and installation
  • Women, Infants and Children (WIC) services
  • Blood lead testing for children
  • Community blood glucose and blood pressure screenings
