The CHR is a liaison between the community and the health center.  The CHR is primarily responsible for planning, developing, and implementing community health services.  The CHR will be the WIC Clerk on three days of the month when WIC services are offered.


All employees of Stockbridge Munsee Community must meet the following qualification:

1.  Must submit to and pass a pre-employment drug and health screening.

2.  Must be at least 18 years of age, unless otherwise stated on job description

3.  Must maintain an acceptable departmental attendance record

4.  Must be able to work weekends, nights and holidays

5.  Must obtain and maintain Elder/Youth License as stated on job description


1.      Coordinate the WIC Program.  Schedule clients, make appointments and reminders, complete monthly reports, maintain WIC file, issue drafts, and intake for certification.

2.      Perform heights, weights, hemoglobin, and blood lead screens on patients in WIC Program.

3.      Enter computer data/reports for appropriate reporting agency as required.

4.      Promote Wisconsin Well Women’s Program and lead cancer prevention education and screening efforts.

5.      Coordinate community Injury Prevention; including but not limited to annual bike rodeo, suicide prevention and community safety fair.

6.      Become instructor certified for Red Cross Babysitting classes; set up and hold yearly babysitting classes.

7.      Assist with classes /educational instruction for Community Health projects.

8.      Assist with annual Head Start Screening.  Provide safety in-services to Head Start program participants.

9.      Assist with monitoring the Lifeline Program and replace nonfunctional units.

10.  Promote the Smoking Cessation Program.  Assist with community wide awareness of the Great American Smoke Out.

11.  Assist with the Elderly CHR Program, provide home visits to the Well Elders, including activities at Ella Besaw Center.

12. Repair Eyeglasses as needed.

13. Transport patients according to established policies and protocols.

14. Obtain and maintain Child Passenger Safety (CPS)/ car seat certification.

15.Perform lice screening and treatment as needed.

16 Assist in classes in Health Promotion/Disease Prevention, including diabetes program when needed.

17. Become certified and assist with food inspections.  Maintain and update Food Handler certification list.

19. Coordinate annual pet vaccination clinic for community cats and dogs.

20. Contribute newspaper articles to the paper for health awareness in the community.

21. Complete all necessary reports and data as required.

22. Must maintain an acceptable departmental attendance record.

23. Must be reliable and prompt when reporting to work.

24. Required to attend job related, in-service, meetings, and training to maintain professional and technical knowledge.

25. Must adhere to tribal law and other applicable laws as well as tribal personnel policies and procedures.

      duties and responsibilities will be subject to change based on organizational needs and/or deemed necessary by the supervisor.

27. May be required to satisfactorily complete an exam or other testing requirement(s) to determine skill proficiency.


1.      A High School Diploma or GED is required.

2.      A completed CHR training is preferred. Current CPR training certification and First Aid Training is preferred. (Employee will be required to complete this training within one year upon hire.)

3.      Must submit to a Criminal Investigation Background Check.

4.      Be self-directed and be able to plan and follow through with plans.

5.      Ability to get along and work with others.

6.      Be interested and dedicated to the promotion of health in the Stockbridge-Munsee Community.

7.      Must have demonstrated ability to maintain satisfactory working record in any prior or current employment.

8.      Must submit and pass a background security check to meet the Elder Youth License Requirements as it pertains to the position and the location of the department prior to and during the course of employment with the Stockbridge-Munsee Community.

9.      Must pass pre-employment drug and health screening.  Must adhere to the Tribe’s Drug and Alcohol Free Workplace Policy during the course of employment.

10.  Must have a valid driver’s license, reliable transportation, and insurance are required.  Must obtain a Wisconsin driver’s license within 30 days of employment if applicant has an out-of-state driver’s license.  Must meet and maintain the eligibility to operate a personal or tribal vehicle under the driver acceptability guidelines as established by Mohican Nation Insurance.

11.  Must be able to work some weekends and evenings.


1. This position requires a lot of sitting with occasionally stoop, kneel, crouch, and lift and/or move up to twenty-five (25) pounds.

2. Evening and/or weekend is required. Extended hours and irregular shifts may be required.

3. Work environment requires excellent personal hygiene due to working in close proximity to others and in an enclosed area.

4. A Tuberculosis (TB) Screening and/or TB Skin Test may be required.
